In the level editor, or while playing levels (like in the normal SMB levels)? I'm assuming you mean in the level editor, but I'll explain both:
To get it in the level editor, you need to place a warp pipe to World 4 without the warp text showing up and go down it WITHOUT being in the level test mode
To get it in the SMB levels, get to the end of 1-2's underground part, jump on the pipe that leads up to the flag, face left, duck, and jump towards the top of the pipe (must be big or fire Mario), then enter the first pipe
