Since it's April 1st, I thought I'd upload a very minimal, kind of joke mod that's pretty hard (but it is real). What this does is replaces Goomba Attack with Hell Mode, which instead of raining Goombas, it... Well download it for yourself, or watch this video below if you wanna ruin the surprise!

You can put a "Fire start" entity at the start of the level. Ok, that make less fire than this mod, but combined with "Fish start" , that can make a level very difficult.
